The aim of the study was to examine the support that advanced-level paramedics need from their supervisors to develop and utilise their competence. Methods: Qualitative essay material was collected from a group of experienced advanced-level paramedics (n =41). The material was analysed with inductive content analysis. Results: Twenty-eight sub-categories emerged, forming 6 categories further leading to the grouping of two main categories: 1) the enablement of human resource (HR) management and leadership; and 2) guidelines on how to operate EMS adequately. Conclusions: Supervisors can and should support advanced-level paramedics in their competence utilisation and development. This can be achieved by enabling professional growth by recognising competencies and generating supportive leadership in EMS. Paramedics would also benefit from clear operative guidelines. Overall, supporting paramedics’ competence utilisation and development is strongly connected with leadership standards in daily practices. Therefore, ambulance nurses (ANs) face difficulties in gaining experience in properly handling children, which can lead to stress for the ANs. A deeper understanding of ANs’ emotions through the course of the care encounter is needed. Aim: To describe how ambulance nurses’ feel and manage their emotions before, during and after a care encounter with a child. Methods: A qualitative approach was adopted, with content analysis being performed on seventeen individual interviews. Findings: Three themes were identified: Feeling worried and insecure, Emotional surge, and Mixed feelings of satisfaction and concern. Conclusion: The ANs experienced a range of emotions during a care encounter with children as they had little experience with it. More training and education in paediatric care could benefit ANs. The ANs use professionalism during care encounters to create a calm and secure atmosphere for families. Their will to bring about a safe journey for the child and family through the care encounter supersedes their nervousness. Reflection after a care encounter could strengthen their confidence in their professional role and reduce emotional distress in future encounters. We describe patients’ ED length of stay (LOS), crowding and experiences of patients with respiratory complaints during the first Covid-19 peak. Methods: Retrospective analysis of demand, ED LOS, crowding, and a patient experience survey during a 12-week period in 2020 and similar periods in 2018 and 2019. Crowding levels were calculated using the National ED OverCrowding Scale. Results: The number of patients with respiratory complaints increased significantly, while total ED numbers were unchanged. Although presentation during the Covid-19 peak and needing hospital admission were associated with a longer ED LOS in patients with respiratory complaints, significantly less crowding occurred compared with the 2018 and 2019 periods. Increased ED LOS was associated with lower patient experience scores. Conclusion: Advanced warning and its associated preparation within the hospital and the community prevented significant delays in ED throughput during the first Covid-19 peak. The aim of this systematic review was to investigate prognostic accuracy in detecting sepsis in the emergency department by comparing the previous sepsis-2 screening tool, the Systemic Inflammatory Response Syndrome (SIRS) and the current sepsis-3 screening tool, the Quick Sequential Organ Failure Assessment (qSOFA). Methods: This systematic review used the guideline by Bettany-Saltikov and McSherry and was reported according to the Preferred Reporting Items for Systematic Reviews and meta-Analyses (PRISMA) 2020 checklist. The protocol was registered in PROSPERO. A systematic search was conducted using the CINAHL, EMBASE and MEDLINE databases. Study selection and risk of bias was performed independently by pair of authors. Results: Five articles were included. Overall, SIRS showed higher sensitivity than qSOFA, while qSOFA showed higher specificity than SIRS. The positive predictive value for qSOFA was superior, while there was a minor deviation in negative predictive value between qSOFA and SIRS. Conclusion: The overall recommendation based on the included studies indicates that qSOFA is the better-suited screening tool for prognostic accuracy in detecting sepsis in the emergency department. Nevertheless, nurses attempted to enhance their communication skills. Organizational support is pertinent to establish effective communication strategies. Therefore, it is important to explore patient perspectives of information exchange and clinical assessment. Aim: To evaluate experiences of care, communication, and teamwork from ED patients’ perspectives. Methods: Semi-structured interviews were conducted with 17 patients who were assessed in a Swedish ED during Spring 2021. Thematic analysis was used. Results: Participants’ experiences reflected the complex environment of the ED. Findings emphasize the importance of information exchange in relation to a caring approach. Three themes emerged: the need for a caring approach by HCPs towards patients’; the need for dialogue between patient and HCPs; and the need for information on ED environment constraints. Conclusions: Patients felt comforted when they experienced a caring empathic approach from the HCPs. For example, patients valued an individual holistic approach rather than feeling that they were being objectified by their medical conditions. This was important in coping with the anxiety caused by a stressful ED environment. There is a critical need for effective exchange of information between patients and HCPs. Title A name given to the resource International Emergency Nursing Vol. 66 January 2023 Physicians and nurse’s perceptions of leadership in military pre-hospital emergency care Description An account of the resource Over the last 20 years, the Swedish Armed Forces have participated in international armed operations in Afghanistan, Bosnia, Kosovo, and Liberia, among others [1]. Physicians and nurses are recruited from the civilian sector, and military physicians and nurses in the Swedish Armed Forces are, by definition, non-combatants. The physicians and nurses are under the command of a tactical officer, and thus the tactical officer has great responsibility for the emergency care on the battlefield. The conditions of military pre-hospital emergency care differ from those of civilian pre-hospital emergency care. One aspect is the difference in environmental conditions and safety, which means that physicians and nurses are exposed to the same risks as regular soldiers [2]. The aim of medical objectives is for the Swedish nurses to treat the injured soldier within 30 min of the injury in the battle zone, which is relatively unique from an international perspective [3]. It is argued that taking care of casualties in the battlefield is a complex task. “Casualty care in austere environments is context dependent. It is not possible to transfer civilian guidelines to the military environment without the necessary adjustments to these guidelines, in order not to cause problems” (2, p.71). In nursing, two leadership styles are commonly used i.e., transactional, and transformational. Transactional leadership is based on the leader following the development of the situation and intervening when something negative happens. This style can be valuable in situations that include lack of time and high levels of stress. Transformational leadership is more about compliance, reward, motivation, and creating responsibility for achieving common goals.
