                <text>Introduction: As the Coronavirus Disease 2019 (COVID-19) cases in Kenya begin to rise, the number of severe and&#13;
critical COVID-19 patients has the potential to quickly overload the local healthcare system beyond its capacity&#13;
to treat people.&#13;
Objective: The purpose of this study was to gather information about the ability of hospitals in Kenya to provide&#13;
emergency and critical care services and to identify priority actions for use by policymakers and other stakeholders&#13;
as a roadmap toward strengthening the COVID-19 response in the country.&#13;
Methods: This was a comprehensive review of the published and grey literature on emergency and critical care&#13;
services in Kenya published in the last three years through April 2020. Screening of articles was conducted&#13;
independently by the authors and the final decision for inclusion was made collaboratively. A total of 15 papers&#13;
and documents were included in the review.&#13;
Key recommendations: There is an urgent need to strengthen prehospital emergency care in Kenya by establishing&#13;
a single toll-free ambulance access number and an integrated public Emergency Medical Services (EMS) system&#13;
to respond to severe and critical COVID-19 patients in the community and other emergency cases. Functional 24-&#13;
h emergency centres (ECs) need to be established in all the level 4, 5 and 6 hospitals in the country to ensure&#13;
these patients receive immediate lifesaving emergency care when they arrive at the hospitals. The ECs should be&#13;
equipped with pulse oximeters and functioning oxygen systems and have the necessary resources and skills to&#13;
perform endotracheal intubation to manage COVID-19-induced respiratory distress and hypoxia. Additional&#13;
intensive care unit (ICU) beds and ventilators are also needed to ensure continuity of care for the critically ill&#13;
patients seen in the EC. Appropriate practical interventions should be instituted to limit the spread of COVID-19&#13;
to healthcare personnel and other patients within the healthcare system. Further research with individual facility&#13;
levels of assessment around infrastructure and service provision is necessary to more narrowly define areas with&#13;
significant shortfalls in emergency and critical care services as the number of COVID-19 cases in the country&#13;
increase.</text>

                <text>Introduction: Globally, length of stay of patients in emergency departments remains a challenge. Remaining in the&#13;
emergency department for &gt;12 h increases health care costs, morbidity and mortality rates and leads to&#13;
crowding and lower patient satisfaction.&#13;
The aim of this research was to describe the areas of delay related to prolonged length of stay in the emergency&#13;
department of an academic hospital.&#13;
Methods: A quantitative retrospective study was done. The Input-Throughput-Output model was used to identify&#13;
the areas of patients’ journey through the emergency department. The possible areas of delay where then&#13;
described. Using systematic sampling, a total of 100 patient files managed in an emergency department of an&#13;
academic hospital in South Africa were audited over a period of 3 months. Descriptive statistics and regression&#13;
analysis was used to analyse data.&#13;
Results: The mean length of stay of patients in the emergency department was 73 h 49 min. The length of stay per&#13;
phase was: input (3 h 17 min), throughput (16 h 25 min) and output (54 h 7 min). A strong significant relationship&#13;
found between the length of stay and the time taken between disposition decision (throughput phase)&#13;
disposition decision to admission or discharge of patients from the ED (output phase) (p &lt; 0.05).&#13;
Conclusion: The output phase was identified as the longest area of delay in this study, with the time taken between&#13;
disposition decision to admission or discharge of patients from the ED (patients waiting for inpatient beds)&#13;
as the main significant area of delay.</text>

                <text>Introduction: Patients’ unscheduled return visits (URVs) to the paediatric emergency Centre (PEC) contribute to&#13;
overcrowding and affect health service delivery and overall quality of care. This study assessed the characteristics&#13;
and outcomes of paediatric patients with URVs (within 72 hours) to the PEC at a private tertiary hospital in&#13;
Kenya.&#13;
Methods: We conducted a retrospective chart review of all URVs within 72 hours among paediatric patients aged&#13;
≤15 years between 1 July and 31 December 2018 at the tertiary hospital in Nairobi, Kenya.&#13;
Results: During the study period, 1.6% (n=172) of patients who visited the PEC returned within 72 hours, with&#13;
4.7% revisiting the PEC more than once. Patients’ median age was 36 months (interquartile range: 42 months);&#13;
over half were male (51.7%), 55.8% were ambulatory and 84.3% were insured. In addition, 21% (n=36) had&#13;
chronic diseases and 7% (n=12) had drug allergies. Respiratory (59.5%) and gastrointestinal (21.5%) tract infections&#13;
were the most common diagnoses. Compared with the first visit, more patients with URVs were classified&#13;
as urgent (1.7% vs. 5.2%) and were non-ambulatory (44.2% vs. 49.5%, p=&lt;0.001); 18% of these patients were&#13;
admitted. Of these 58% were male, 83.9% were aged 0–5 years, 12.9% were classified as urgent, 64.5% had&#13;
respiratory tract infections and 16.1% had gastrointestinal tract infections. Being admitted was associated with&#13;
patient acuity (p=0.004), laboratory tests (p=&lt;0.001) and ambulatory status (p=0.041).&#13;
Conclusion: The URV rate is low in our setting. Patients who returned to the PEC within 72 hours tended to be&#13;
male, under 5 years old and insured. Many were non-urgent cases with diagnoses of respiratory and gastrointestinal&#13;
tract infections. The findings suggest that some URVs were necessary and may have contributed to better&#13;
care and improved outcomes while others highlight a need for effective patient education and comprehensive&#13;
initial assessment.</text>

                <text>Introduction: Weight estimation in children is critical in paediatric emergencies. The Broselow Tape (BT) and&#13;
most age-based formulae for weight estimation were derived in high-income countries and are thought to&#13;
overestimate the weight of children in low-income countries. This study sought to validate the 2017 BT, and&#13;
eight age-based weight estimation formulae among Ghanaian children and to derive a weight estimation formula&#13;
using this data.&#13;
Methods: A cross-sectional study was conducted in the Tamale Teaching Hospital (TTH) in Ghana. Children aged&#13;
between 2 months and 13 years had their weights estimated by the 2017 BT and eight age-based formulae. These&#13;
estimated weights were compared to the weight of the children measured by a calibrated Seca scale using mean&#13;
percentage error (MPE) and the percentage of weight estimates within 10% and 20% of actual weight. Bland-&#13;
Altman method was used to assess agreement between estimated and actual weight of the children. A new&#13;
formula was derived by linear regression.&#13;
Results: Seven hundred and seventy-five children took part in the study. The 2017 BT, Original APLS (APLS1) and&#13;
Nelson’s formulae performed best with proportion of weight estimates within 10% of actual weight being 47.5%,&#13;
51.1% and 47.5% respectively. The formula developed in this study was: WE = 3Am / 10 + 5 (for infants &lt;12&#13;
months), WE = 2A + 7 (1 to 4 years) and WE = 2A + 9 (5 to 13 years), where WE is estimated weight, Am is age in&#13;
completed months and A is age in completed years. The new formula had similar accuracy as the three best&#13;
performing methods in this study.&#13;
Conclusion: The Broselow Tape, APLS1 and the Nelson’s formula were the most accurate in this study. APLS1 and&#13;
the Broselow Tape can be used for weight estimation in Ghanaian children when no other better method is&#13;
available.</text>

                <text>Introduction: Traumatic injuries and their resulting mortality and disability impose a disproportionate burden on&#13;
sub-Saharan countries like Rwanda. An important facet of addressing injury burdens is to comprehend injury&#13;
patterns and aetiologies of trauma. This study is a cross-sectional analysis of injuries, treatments and outcomes at&#13;
the University Teaching Hospital-Kigali (CHUK).&#13;
Methods: A random sample of Emergency Centre (EC) injury patients presenting during August 2015 through July&#13;
2016 was accrued. Patients were excluded if they had non-traumatic illness. Data included demographics,&#13;
clinical presentation, injury type(s), mechanism of injury, and EC disposition. Descriptive statics were utilised to&#13;
explore characteristics of the population.&#13;
Results: A random sample of 786 trauma patients met inclusion criteria and were analysed. The median age was&#13;
28 (IQR 6–50) years and 69.4% were male. Of all trauma patients 49.4% presented secondary to road traffic&#13;
injuries (RTIs), 23.9% due to falls, 10.9% due to penetrating trauma. Craniofacial trauma was the most frequent&#13;
traumatic injury location at 36.3%. Lower limb trauma and upper limb trauma constituted 35.8% and 27.1% of&#13;
all injuries. Admission was required in 68.2% of cases, 23.3% were admitted to the orthopaedic service with the&#13;
second highest admission to the surgical service (19.2%). Of those admitted to the hospital, the median LOS was&#13;
6 days (IQR 3–14), in the subset of patients requiring operative intervention, the median LOS was also 6 days&#13;
(IQR 3–16). Death occurred in 5.5% of admitted patients in the hospital.&#13;
Conclusion: The traumatic injury burden is borne more proportionally by young males in Kigali, Rwanda. Blunt&#13;
trauma accounts for a majority of trauma patient presentations; of these RTIs constitute nearly half the injury&#13;
mechanisms. These findings suggest that this population has substantial injury burdens and prevention and care&#13;
interventions focused in this demographic group could provide positive impacts in the study setting</text>

                <text>Effective training of lay people in cardiopulmonary resuscitation&#13;
(CPR) is recognised as an important contributor to increased survival&#13;
rates from cardiac arrest. However, owing to high risk of COVID-19, it is&#13;
currently recommended to refrain from traditional instructor-led&#13;
resuscitation training. As an alternative, individual self-directed&#13;
learning is advisable [1]. Self-instruction programs utilising video or&#13;
on-line learning with synchronous or asynchronous hands-on practice&#13;
were shown to be at least as effective as classroom training both in&#13;
producing necessary skills and improving willingness of trainees to&#13;
attempt CPR.&#13;
For practicing resuscitation skills in individual environment, a&#13;
manikin is required. Whereas simplified inflatable CPR manikins suitable&#13;
for home use are currently available on market, a need to spend&#13;
time and money to buy the manikin may limit motivation to initiate or&#13;
complete the training. An interesting option for those who want to get a&#13;
dummy right away and at no cost is the creation of a homebrew manikin.</text>

                <text>Introduction: Trauma is among the leading causes of death and disability in both adults and children worldwide.&#13;
In Malawi, trauma patients are commonly brought in dead (BID). We aimed to describe the prevalence, sociodemographic,&#13;
and injury-related characteristics of patients BID to Kamuzu Central Hospital (KCH), a referral&#13;
hospital in Lilongwe, Malawi.&#13;
Methods: We retrospectively reviewed records of all patients BID in the trauma surveillance registry at KCH from&#13;
February 2008 to September 2019. We excluded patients BID that did not present to the emergency centre, and&#13;
were instead taken to the mortuary directly. We used descriptive statistics to evaluate the epidemiology of patients&#13;
BID.&#13;
Results: We reviewed 106,198 trauma records and 1889 (1.8%) were BID patients. Most patients BID were male,&#13;
in both adult (n = 1337/1528, 88.4%) and children (n = 231/360, 64.9%) cohorts. The mean age was 34.7 (SD&#13;
11.9) years in adults and 7.8 (SD 5.4) years in children. Among the adult BID patients, 33.2% were unemployed,&#13;
25.6% were construction workers, and 10.1% were small business owners or managers. The common injury&#13;
mechanisms in adults were road traffic-related injuries (RTIs) (47.1%) and assaults (23.6%). In children, injuries&#13;
resulted from RTIs (39.7%), with 74.4% of those were pedestrians hit by cars, drowning (22.9%), and burns&#13;

Conclusion: Efforts to reduce prehospital trauma mortality must focus on improving prehospital care, including&#13;
training the police and community in basic life support and improving resources towards prehospital trauma&#13;
