<text>Introduction: Mortality and morbidity from Non-Communicable Diseases (NCDs) in Africa are expected to worsen
if the status quo is maintained. Emergency care settings act as a primary point of entry into the health system for
a spectrum of NCD-related illnesses, however, there is a dearth of literature on this population. We conducted a
systematic review assessing available evidence on epidemiology, interventions and management of NCDs in
acute and emergency care settings in Kenya, the largest economy in East Africa and a medical hub for the
continent.
Methods: All searches were run on July 15, 2015 and updated on December 11, 2020, capturing concepts of
NCDs, and acute and emergency care. The study is registered at PROSPERO (CRD42018088621).
Results: We retrieved a total of 461 references, and an additional 23 articles in grey literature. 391 studies were
excluded by title or abstract, and 93 articles read in full. We included 10 articles in final thematic analysis. The
majority of studies were conducted in tertiary referral or private/mission hospitals. Cancer, diabetes, cardiovascular
disease and renal disease were addressed. Majority of the studies were retrospective, cross-sectional in
design; no interventions or clinical trials were identified. There was a lack of access to basic diagnostic tools, and
management of NCDs and their complications was limited.
Conclusion: There is a paucity of literature on NCDs in Kenyan emergency care settings, with particular gaps on
interventions and management. Opportunities include nationally representative, longitudinal research such as
surveillance and registries, as well as clinical trials and implementation science to advance evidence-based,
context-specific care.</text>

<text>Background: Traumatic injuries are proportionally higher in low- and middle-income countries (LMICs) than
high-income counties. Data on trauma epidemiology and patients’ outcomes are limited in LMICs.
Methods: A retrospective review of medical records was performed for trauma admissions to the Princess Marina
Hospital general surgical (GS) wards from August 2017 to July 2018. Data on demographics, mechanisms of
injury, body parts injured, Revised Trauma Score, surgical procedures, hospital stay, and outcomes were
analysed.
Results: During the study period, 2610 patients were admitted to GS wards, 1307 were emergency admissions.
Trauma contributed 22.1% (576) of the total and 44.1% of the emergency admissions. Among the trauma admissions,
79.3% (457) were male. The median[interquartile range(IQR)](range) age in years was 30[24–40]
(13–97). The main mechanisms of injury were interpersonal violence (IPV), 53.1% and road traffic crashes
(RTCs), 23.1%. More females than males suffered animal bites (5.9% vs. 0.9%), and burns (8.4% vs. 4.2%), while
more males than females were affected by IPV (57.8% vs. 35.3%) and self-harm (5.5% vs. 3.4%). Multiple body
parts were injured in 6.6%, mainly by RTCs. Interpersonal violence (IPV) and RTCs resulted in significant
numbers of head and neck injuries, 57.3% and 22.2% respectively. More females than males had multiple bodyparts
injury 34.5% vs. 18.5%. Revised Trauma Score (RTS) of ≤11 was recorded in IPV, 38.4% and RTCs, 33.6%.
Surgical procedures were performed on 44.4% patients. The most common surgical procedures were laparotomy
(27.8%), insertion of chest tube (27.8%), and craniotomy/burr hole(25.1%). Complications were recorded in
10.1% of the patients(58) including 39 deaths, 6.8% of the 576.
Conclusion: Trauma contributed significantly to the total GS and emergency admissions. The most common
mechanism of injury was IPV with head and neck the most frequently injured body part. Further studies on IPV
and trauma admissions involving paediatric and orthopaedic patients are warranted.</text>

<text>The objective of this study was to compare the effect of the Covid-19 lockdown and the alcohol restriction on the
number of cases that presented to the Emergency Department (ED) with the same time period two years prior.
The method used was a retrospective review of medical records, directly comparing the types and numbers of
trauma cases as well as non-trauma cases that presented to the ED in March and April 2020 with the same period
two years prior. Our results showed a reduction during both months of lockdown compared to the same time
period in 2018 with trauma cases in March 2020 down 33.14% and April 2020 down 57.93%. The non-trauma
ED cases were down 2.52% in March and 37.43% in April compared to two years prior. When comparing only the
last 6 days of March, a significant percentage decrease is visible as trauma cases fell from 20.79% in 2018 to just
8.58% of the total cases in 2020. In conclusion, our data showed a significant reduction in almost all types of ED
cases during the lockdown period, but most significantly in trauma-related cases which was likely due, inter alia,
to the prohibition of alcohol sales, gatherings and unnecessary travel.</text>

<text>Introduction: As the Coronavirus Disease 2019 (COVID-19) cases in Kenya begin to rise, the number of severe and
critical COVID-19 patients has the potential to quickly overload the local healthcare system beyond its capacity
to treat people.
Objective: The purpose of this study was to gather information about the ability of hospitals in Kenya to provide
emergency and critical care services and to identify priority actions for use by policymakers and other stakeholders
as a roadmap toward strengthening the COVID-19 response in the country.
Methods: This was a comprehensive review of the published and grey literature on emergency and critical care
services in Kenya published in the last three years through April 2020. Screening of articles was conducted
independently by the authors and the final decision for inclusion was made collaboratively. A total of 15 papers
and documents were included in the review.
Key recommendations: There is an urgent need to strengthen prehospital emergency care in Kenya by establishing
a single toll-free ambulance access number and an integrated public Emergency Medical Services (EMS) system
to respond to severe and critical COVID-19 patients in the community and other emergency cases. Functional 24-
h emergency centres (ECs) need to be established in all the level 4, 5 and 6 hospitals in the country to ensure
these patients receive immediate lifesaving emergency care when they arrive at the hospitals. The ECs should be
equipped with pulse oximeters and functioning oxygen systems and have the necessary resources and skills to
perform endotracheal intubation to manage COVID-19-induced respiratory distress and hypoxia. Additional
intensive care unit (ICU) beds and ventilators are also needed to ensure continuity of care for the critically ill
patients seen in the EC. Appropriate practical interventions should be instituted to limit the spread of COVID-19
to healthcare personnel and other patients within the healthcare system. Further research with individual facility
levels of assessment around infrastructure and service provision is necessary to more narrowly define areas with
significant shortfalls in emergency and critical care services as the number of COVID-19 cases in the country
increase.</text>

<text>Introduction: Globally, length of stay of patients in emergency departments remains a challenge. Remaining in the
emergency department for >12 h increases health care costs, morbidity and mortality rates and leads to
crowding and lower patient satisfaction.
The aim of this research was to describe the areas of delay related to prolonged length of stay in the emergency
department of an academic hospital.
Methods: A quantitative retrospective study was done. The Input-Throughput-Output model was used to identify
the areas of patients’ journey through the emergency department. The possible areas of delay where then
described. Using systematic sampling, a total of 100 patient files managed in an emergency department of an
academic hospital in South Africa were audited over a period of 3 months. Descriptive statistics and regression
analysis was used to analyse data.
Results: The mean length of stay of patients in the emergency department was 73 h 49 min. The length of stay per
phase was: input (3 h 17 min), throughput (16 h 25 min) and output (54 h 7 min). A strong significant relationship
found between the length of stay and the time taken between disposition decision (throughput phase)
disposition decision to admission or discharge of patients from the ED (output phase) (p < 0.05).
Conclusion: The output phase was identified as the longest area of delay in this study, with the time taken between
disposition decision to admission or discharge of patients from the ED (patients waiting for inpatient beds)
as the main significant area of delay.</text>

Results: During the study period, 1.6% (n=172) of patients who visited the PEC returned within 72 hours, with
4.7% revisiting the PEC more than once. Patients’ median age was 36 months (interquartile range: 42 months);
over half were male (51.7%), 55.8% were ambulatory and 84.3% were insured. In addition, 21% (n=36) had
chronic diseases and 7% (n=12) had drug allergies. Respiratory (59.5%) and gastrointestinal (21.5%) tract infections
were the most common diagnoses. Compared with the first visit, more patients with URVs were classified
as urgent (1.7% vs. 5.2%) and were non-ambulatory (44.2% vs. 49.5%, p=<0.001); 18% of these patients were
admitted. Of these 58% were male, 83.9% were aged 0–5 years, 12.9% were classified as urgent, 64.5% had
respiratory tract infections and 16.1% had gastrointestinal tract infections. Being admitted was associated with
patient acuity (p=0.004), laboratory tests (p=<0.001) and ambulatory status (p=0.041).
Conclusion: The URV rate is low in our setting. Patients who returned to the PEC within 72 hours tended to be
male, under 5 years old and insured. Many were non-urgent cases with diagnoses of respiratory and gastrointestinal
tract infections. The findings suggest that some URVs were necessary and may have contributed to better
care and improved outcomes while others highlight a need for effective patient education and comprehensive
initial assessment.</text>

<text>Introduction: Weight estimation in children is critical in paediatric emergencies. The Broselow Tape (BT) and
most age-based formulae for weight estimation were derived in high-income countries and are thought to
overestimate the weight of children in low-income countries. This study sought to validate the 2017 BT, and
eight age-based weight estimation formulae among Ghanaian children and to derive a weight estimation formula
using this data.
Methods: A cross-sectional study was conducted in the Tamale Teaching Hospital (TTH) in Ghana. Children aged
between 2 months and 13 years had their weights estimated by the 2017 BT and eight age-based formulae. These
estimated weights were compared to the weight of the children measured by a calibrated Seca scale using mean
percentage error (MPE) and the percentage of weight estimates within 10% and 20% of actual weight. Bland-
Altman method was used to assess agreement between estimated and actual weight of the children. A new
formula was derived by linear regression.
Results: Seven hundred and seventy-five children took part in the study. The 2017 BT, Original APLS (APLS1) and
Nelson’s formulae performed best with proportion of weight estimates within 10% of actual weight being 47.5%,
51.1% and 47.5% respectively. The formula developed in this study was: WE = 3Am / 10 + 5 (for infants <12
months), WE = 2A + 7 (1 to 4 years) and WE = 2A + 9 (5 to 13 years), where WE is estimated weight, Am is age in
completed months and A is age in completed years. The new formula had similar accuracy as the three best
performing methods in this study.
Conclusion: The Broselow Tape, APLS1 and the Nelson’s formula were the most accurate in this study. APLS1 and
the Broselow Tape can be used for weight estimation in Ghanaian children when no other better method is
available.</text>
