<text>Stress among healthcare workers is important
to note because it is one of the fundamentals to
implementing optimal service facilities, i.e. hospitals.1
The coronavirus disease 2019 (COVID-19) pandemic has
brought certain stressors due to increased workload,
as well as greater vigilance and adherence to work.
However, healthcare workers are still obliged to
perform their duties amidst fear of being infected with
COVID-19, which can trigger and/or aggravate stress.2–
5
In Indonesia, 55% of healthcare workers experienced

stress, 65.8% anxiety, and 23.5% depression during the
COVID-19 pandemic.6 According to a study in Jordan,
35% of healthcare workers experienced severe stress
during the COVID-19 pandemic.7 Previous studies
have only assessed stressful conditions in healthcare
workers without exploring the stressors; thus, this
study used a stressor questionnaire to identify the
alteration of stressors among healthcare workers
associated with the stress condition during the
COVID-19 pandemic.</text>

<text>Premature rupture of membranes (PROM) is
defined as a spontaneous rupture of membranes any
time after the 28th week of pregnancy but before the
onset of labor. It occurs in 5–10% of all pregnancies,
contributing to 30–40% of preterm labor cases (15
million cases per year),1

which increases mortality and
morbidity for mother, fetus, and neonate.2,3 PROM
arises from complex, multifaceted pathophysiologic
pathways where the inflammation axis plays a crucial
role.4

Recently, an increasing number of studies have
shown the implication of sterile inflammation in
the fetal membrane weakening of PROM.5,6 This
concept is dependent on specific molecules called
alarmins or damage-associated molecular patterns
(DAMP), which are released and recognized by
pattern recognition receptors (PRRs), leading to a
microbial-free inflammatory response or a sterile
inflammation.1

condition of pelvic floor muscle weakness, most
commonly caused by a vaginal birth process.1
More
than 46% of women with a history of vaginal birth have
PFD. PFD occurs due to multiple structure disruptions,
including levator ani muscles (LAMs), anal sphincter,
and perineum, and causes symptoms of urinary and

fecal incontinence.2 LAM defect is classified into macro-
and microtrauma. Macrotrauma, also called avulsion,

is a complete detachment of LAM from the inferior

pubic ramus. Meanwhile, microtrauma, also called
ballooning, is an over distention of the LAM. Both are
two independent risk factors for PFD and lead to pelvic
organ prolapse (POP) and stress urinary incontinence
(SUI).3 The prevalence of SUI 3 months after delivery
in Indonesia was estimated at 20.1%, while primiparous
women with vaginal delivery had a 2.1 times higher risk
of developing SUI.4

Unfortunately, postpartum SUI
treatment is still limited to lifestyle and pelvic floor
rehabilitation.5</text>

<text>Global cognitive impairment is an important
complication of epilepsy. The prevalence of
cognitive impairment in patients with epilepsy is
currently estimated to be 70–80%, but this can vary
by region.1,2 The number of patients with epilepsy in
developing countries tends to be higher than that of
developed countries, and only a small portion of the
patient population receives appropriate treatment;
thus, it can be estimated that the prevalence of
global cognitive impairment will be much higher in
developing countries.3,4 Our previous study in the
same population who received the conventional
antiepileptic drug (AED) monotherapy showed that

the prevalence of cognitive impairment was 75%.5
However, the prevalence of epilepsy-associated
cognitive disorders in other parts of Indonesia is
still rare. Studies conducted in Aceh and Palembang
with small sample sizes showed a prevalence of

20.6–69%.6,7 If not appropriately treated, epilepsy-
associated cognitive impairment can progress to

dementia, leading to decreased quality of life and
increased socioeconomic and health burden on their
families and the government.2
Global cognitive impairment in patients with
epilepsy can occur at the onset of epilepsy or years
after the initial diagnosis.8,9</text>

<text>A foreign body in the respiratory tract is rare and
is mostly associated with aspiration events.1,2 However,
foreign bodies in the trachea are common, especially in
children, and are mainly caused by aspiration. Although
penetrating tracheal injuries have been frequently
documented, bullet components remaining in the
airway are rarely reported. After maxillofacial or chest
trauma, bullet migration to the airway occurs in some
situations. Tracheal damage is usually not difficult
to diagnose. Respiratory discomfort, hemoptysis,
air escaping from the incision, and subcutaneous
emphysema are the possible symptoms. The airway
injury might be minor or fatal, and local trauma such as

vascular and esophageal injuries may impact mortality
and morbidity.3,4
A foreign body in the respiratory tract due to a
ballistic projectile is an extremely rare case. It is usually
sterile due to its physical characteristics. However,
it can develop into a life-threatening condition if left
untreated. The migration of the foreign body is a
common case in the upper airway system, judging
by the size of the lumen. It is extremely rare to find a
migrating foreign body once it is located in the distal
airway tract, namely after the tracheal bifurcation. In
a common situation, the lodging of the foreign body in
the lower airway tract prevents them from dislodging.</text>

neck cancer poses a challenge for both surgeons and
oncologists due to its high risk, particularly when it
manifests following multiple treatments. Attempting
surgical salvage in this population adds another layer
of complexity. An aggressive surgical approach to
attain oncological clearance is a challenge that head
and neck surgeons must face. Radical en bloc resection
is the aim of curative intention.
Controversies arise between the survival benefit
and postoperative morbidity and mortality. A previous
study indicated that in advanced squamous cell
carcinoma involving the carotid artery, surgically
treated patients with curative intent had improved

overall survival compared with the palliative group.
However, the survival outcome was not significantly
different between those treated with carotid artery
resection with or without reanastomosis, curative
peeling with or without adjuvant therapy, and
definitive chemoradiation therapy.1

Another study
reported that overall survival was better in patients
with carotid resection and reconstruction with low
perioperative neuromorbidity and mortality rates.2–4
Although the procedure is now well known, studies
about carotid artery resection and reconstruction as a
part of salvage treatment are still limited. Moreover,
this procedure requires careful consideration for
those who have undergone radiotherapy and multiple</text>

<text>BACKGROUND Extracorporeal shock wave lithotripsy (ESWL) is one of the first-line
treatment options for patients with renal stones <2 cm. The large variability in ESWL
results may be due to the stone visualization methods using ultrasonography (USG),
fluoroscopy, or a combination of both. This study aimed to review the efficacy and safety
of the stone visualization method on the stone-free rate (SFR) and postprocedural
complications in nephrolithiasis patients.
METHODS We conducted a systematic review of USG and fluoroscopy on ESWL until

July 2022, based on the Preferred Reporting Items for Systematic Reviews and Meta-
Analyses guidelines. We assessed and collected summaries of the screened papers.

The main outcomes assessed were the SFR of renal stones and postprocedural
complications between imaging modalities.
RESULTS A total of 7 studies were assessed, including 6 comparative assessments
of USG versus fluoroscopy and 1 comparative assessment of USG and fluoroscopy
versus fluoroscopy only. Although all studies showed that USG had a higher SFR than
fluoroscopy, only 1 study showed a significant difference (p = 0.008). Additionally,
superior results were obtained using a combination of USG and fluoroscopy compared
with fluoroscopy only. Most studies agreed that USG was not inferior in post-ESWL
complication results.
CONCLUSIONS Overall, the use of USG is comparable to fluoroscopy because it does
not provide a significant difference in the SFR and complications. In most cases, USG
is preferred because of the absence of radiation. The combination of fluoroscopy and
USG also provides more promising results than a single modality.</text>

<text>BACKGROUND Rheumatic heart disease (RHD) is one of the most common
cardiovascular problems in Indonesia. Comprehensive data regarding patient
characteristics are critical in planning optimal treatment strategies to relieve the
burden of RHD. This study aimed to describe the clinical and echocardiographic
characteristics of patients across several types of valvular lesions in RHD in the
Indonesian population.
METHODS This retrospective study was performed between January 2016 and June
2019 at the National Cardiovascular Center Harapan Kita, Jakarta, Indonesia. The study
population comprised all patients with significant valve disease aged ≥18 years. Patient
characteristics and echocardiographic parameters were collected retrospectively from
medical records and hospital information systems. Patients were classified into several
groups based on etiologies of valve disease.
RESULTS Of 5,482 patients with significant valve lesions, 2,333 (42.6%) were RHD
patients. They were predominantly female (64.1%) and younger (mean [standard
deviation] age 42.61 [12.01] years). Atrial fibrillation (AF) was the most frequent rhythm
disorder observed in RHD (65.4%). Isolated mitral stenosis was the most common valve
lesion in RHD patients (46.5%). Most patients with RHD had preserved left ventricular
(LV) ejection fraction. Half of the patients with mitral stenosis had reduced right
ventricular (RV) contractility (tricuspid annular plane systolic excursion <17 mm).
CONCLUSIONS Isolated mitral stenosis was the most observed condition of valve
lesions in RHD. Characteristics of RHD patients in this study were predominantly female,
younger age, had preserved LV function, reduced RV function, and high prevalence of
AF.</text>

<text>BACKGROUND Pulmonary resident physicians are part of the frontline workers for
COVID-19 in the hospital and might require a suitable coping strategy to manage stress.
Here, we described their mental health status and coping strategy during the COVID-19
pandemic.
METHODS A cohort study was done to evaluate stress, anxiety, depression, and
coping strategies among pulmonary resident physicians in a tertiary referral hospital.
Questionnaires were administered in 4 time points between August 2020 and June
2021. The DASS-21 questionnaire measured depression, anxiety, and stress, while Brief
COPE instrument qualified the coping strategy.
RESULTS The total respondents who filled the questionnaire in 4 time points in August
2020, October 2020, February 2021, and June 2021 were 60, 93, 96, and 94 resident
physicians with mean stress levels of 4.3, 6.1, 5.4, and 6.3, respectively. Depression,
stress, and anxiety were less frequent during the study period, mostly mild or moderate.
However, moderate anxiety slightly increased in June 2021 when the pandemic reached
its peak. Most of them (95.8%) had effective coping strategies related to religious
beliefs.
CONCLUSIONS Effective coping strategies might preserve the pulmonary resident
physicians’ mental health during the pandemic. Furthermore, the sociodemographic
and cultural background might affect the coping strategy. Practicing religion and
praying or meditating might be essential factors in coping with psychological distress
and a key to adapt to the COVID-19 pandemic.</text>

<text>BACKGROUND The short version of the new brief job stress questionnaire (SV-NBJSQ)
that is originally in Japanese could evaluate work-related stress in Indonesian office
workers. However, it has not been validated in Indonesian. Thus, this study aimed to
test the validity and reliability of the Indonesian version of the SV-NBJSQ.
METHODS This study was conducted by linguistic and cultural adaptation of the
original Japanese questionnaire into Indonesian and continued with exploratory factor
analysis, internal consistency, and reliability test of Cronbach’s alpha. Electronic and
paper-based data were collected using total sampling from office workers at two
companies in Jakarta from December 2017 to August 2018.
RESULTS A total of 438 respondents filled the questionnaire. The final version of the
questionnaire consisted of 63 items, screened based on a loading factor of >0.4 and
13 factors (dimensions). Each factor had a Cronbach’s alpha of 0.628–0.887, while the
entire 63 had 0.904 with a total documented cumulative variation of 64.97%.
CONCLUSIONS The Indonesian version of the SV-NBJSQ is valid and reliable. Thus, we

may use it as a more suitable, updated, and comprehensive tool to evaluate work-
related stress among office workers.
