For example, the cost-effectiveness between sildenafil and the readily available beraprost in the universal health coverage system (Jaminan Kesehatan Nasional [JKN]) is being studied and discussed in the technology screening division of the Ministry of Health. Sildenafil is currently included in the JKN coverage for treating patients with pulmonary hypertension due to better outcomes. Several papers have also called for Indonesia to incorporate international guideline- approved medications for pulmonary hypertension other than beraprost into our regimen.2,3 Furthermore, the prospects for the organ transplantation network and system in Indonesia are being studied to overcome the soaring cost of treatment of end-stage kidney disease using hemodialysis and end-stage heart failure using medications.4 Considerations are also made on transplantation-bridging therapy in end-stage heart failure using a left ventricular assist device. In addition, medical publications are used at the policy-making level, not only for innovations in medical policy but also to improve the existing policy. This is the case for the current investigation of the prospect of using locally made coronary stents instead of imported coronary stents to help suppress costs in coronary heart disease treatment using stent implantation. This approach was advocated by one publication in the Journal of the American College of Cardiology.5,6 Keeping down costs of care means that more people can be treated using the same amount of budget. With the high availability of medical information nowadays, policymakers require solid evidence and engage in lengthy discussions before making a decision. Keeping up with advances in the medical world, the Ministry of Health, with its various divisions, are adapting between the ever-changing advances in the medical world and the country’s need. These changes are studied by the Research and Development Institute (Litbangkes) and the health technology assessment of the Ministry of Health. As the fourth most populated country in the world with an extensive maritime frontier, public health policy in Indonesia needs to be thoroughly discussed to meet the needs of all its citizens. Therefore, the research and development division and the health technology assessment at the Ministry of Health hold monthly briefing regarding public health policy and study articles from various medical journals, either international or Indonesian, for their impact on our public health policy. During the current digital era, physicians are faced with easy access to information worldwide. The onset of the COVID-19 pandemic changes our lifestyle, including our behaviors in seeking information about medicine. 7,8 Evidence-based medicine has always been utilized to avoid errors on clinical decision and health- related policymaking. Currently, physicians can easily access valid and reputable guidelines and studies for all medical conditions from medical journals worldwide. These journals are kept to standard by stringent editorial policies and boards to avoid disseminating wrong information. Medical Journal of Indonesia (MJI), with its global readership, needs to improve the citation index to be more influential in the health-related policy-making process since many Indonesian medical research is published in the journal. In doing so, the journal’s editorial board needs to reach out to editors of medical journals overseas to study ways to improve the journal. Although penetrating tracheal injuries have been frequently documented, bullet components remaining in the airway are rarely reported. After maxillofacial or chest trauma, bullet migration to the airway occurs in some situations. Tracheal damage is usually not difficult to diagnose. Respiratory discomfort, hemoptysis, air escaping from the incision, and subcutaneous emphysema are the possible symptoms. The airway injury might be minor or fatal, and local trauma such as vascular and esophageal injuries may impact mortality and morbidity.3,4 A foreign body in the respiratory tract due to a ballistic projectile is an extremely rare case. It is usually sterile due to its physical characteristics. However, it can develop into a life-threatening condition if left untreated. The migration of the foreign body is a common case in the upper airway system, judging by the size of the lumen. It is extremely rare to find a migrating foreign body once it is located in the distal airway tract, namely after the tracheal bifurcation. In a common situation, the lodging of the foreign body in the lower airway tract prevents them from dislodging. Comprehensive data regarding patient characteristics are critical in planning optimal treatment strategies to relieve the burden of RHD. This study aimed to describe the clinical and echocardiographic characteristics of patients across several types of valvular lesions in RHD in the Indonesian population. METHODS This retrospective study was performed between January 2016 and June 2019 at the National Cardiovascular Center Harapan Kita, Jakarta, Indonesia. The study population comprised all patients with significant valve disease aged ≥18 years. Patient characteristics and echocardiographic parameters were collected retrospectively from medical records and hospital information systems. Patients were classified into several groups based on etiologies of valve disease. RESULTS Of 5,482 patients with significant valve lesions, 2,333 (42.6%) were RHD patients. They were predominantly female (64.1%) and younger (mean [standard deviation] age 42.61 [12.01] years). Atrial fibrillation (AF) was the most frequent rhythm disorder observed in RHD (65.4%). Isolated mitral stenosis was the most common valve lesion in RHD patients (46.5%). Most patients with RHD had preserved left ventricular (LV) ejection fraction. Half of the patients with mitral stenosis had reduced right ventricular (RV) contractility (tricuspid annular plane systolic excursion <17 mm). CONCLUSIONS Isolated mitral stenosis was the most observed condition of valve lesions in RHD. Characteristics of RHD patients in this study were predominantly female, younger age, had preserved LV function, reduced RV function, and high prevalence of AF. The large variability in ESWL results may be due to the stone visualization methods using ultrasonography (USG), fluoroscopy, or a combination of both. This study aimed to review the efficacy and safety of the stone visualization method on the stone-free rate (SFR) and postprocedural complications in nephrolithiasis patients. METHODS We conducted a systematic review of USG and fluoroscopy on ESWL until July 2022, based on the Preferred Reporting Items for Systematic Reviews and Meta- Analyses guidelines. We assessed and collected summaries of the screened papers. The main outcomes assessed were the SFR of renal stones and postprocedural complications between imaging modalities. RESULTS A total of 7 studies were assessed, including 6 comparative assessments of USG versus fluoroscopy and 1 comparative assessment of USG and fluoroscopy versus fluoroscopy only. Although all studies showed that USG had a higher SFR than fluoroscopy, only 1 study showed a significant difference (p = 0.008). Additionally, superior results were obtained using a combination of USG and fluoroscopy compared with fluoroscopy only. Most studies agreed that USG was not inferior in post-ESWL complication results. CONCLUSIONS Overall, the use of USG is comparable to fluoroscopy because it does not provide a significant difference in the SFR and complications. In most cases, USG is preferred because of the absence of radiation. The combination of fluoroscopy and USG also provides more promising results than a single modality.
