                <text>Background: Redundant pathology testing is a significant quality issue in healthcare with the capacity to lead to unnecessary costs, resource &#13;
strain, environmental waste, and potential patient harm. This study aimed to assess the prevalence of potentially unnecessary repeat full blood &#13;
count (FBC), electrolytes urea creatinine (EUC), and calcium magnesium phosphate (CMP) testing within 24h in New South Wales (NSW) hospitals &#13;
and to identify associated demographic and clinical factors.&#13;
Methods: A multicentre retrospective observational study was conducted across eight public hospitals in two NSW local health districts Australia, &#13;
from January 2020 to December 2021. Data from the Laboratory Information System, Emergency Department Data Collection, and Admitted &#13;
Patient Data Collection were used. Chi-squared statistics and multivariable logistic regression were used to identify factors associated with &#13;
redundant testing.&#13;
Results: Of the total patient cohort, 35% had a repeat FBC, 33% had a repeat CMP, and 35% had a repeat EUC test &lt;24h. Younger patients &#13;
(&lt;18years) had higher odds of a repeat test &lt;24h compared to older age groups. Significant variability was observed across hospitals, with &#13;
Hospitals C, D, and G exhibiting higher odds of repeat testing. Patients with emergency department encounters had higher odds of repeat tests &#13;
compared to inpatient settings. Additionally, higher socioeconomic status was associated with increased repeat testing.&#13;
Conclusions: Our study revealed that there is a substantial prevalence of potentially unnecessary repeat pathology testing in NSW hospitals, &#13;
with variations across demographic, institutional, and clinical factors. These findings highlight the need for targeted interventions to optimize test &#13;
ordering practices.</text>

                <text>Background: Ambulatory plastic surgery has emerged as a safe and efficient alternative to hospital-based care, particularly in underserved areas &#13;
like Northern Ontario. This study aimed to evaluate the short-term outcomes of plastic surgery procedures performed in a Northern Ontario &#13;
ambulatory office setting.&#13;
Methods: A retrospective chart review was conducted for 505 procedures, performed from August 2023 to August 2024, at one plastic surgery &#13;
clinic. Patient factors, including age, gender, smoking status, use of blood thinners, primary diagnosis, and referring provider, were collected. &#13;
Along with surgical outcomes, including procedure type, complications, and urgent follow-up visits within 2 weeks of operation. Descriptive &#13;
evaluation was used to evaluate outcome trends.&#13;
Results: The majority of patient referrals came from primary care physicians for a wide range of procedures. The most common procedures &#13;
performed in the clinic were skin cancer biopsies and carpal tunnel release. The cohort included 505 patients aged 16 to 100 years (median &#13;
age 64), with 48.7% male, 51.1% female, and 0.2% unspecified. There was an overall incidence of 8% for postoperative complications, with &#13;
infections being the most frequent. Complication rates varied by procedure, with carpal tunnel release showing the lowest rate of 1.5%, and &#13;
punch biopsies showing the highest rate at 11.6% which decreased after a procedural change. The majority of complications arose among the &#13;
70-80 age range.&#13;
Conclusions: This study demonstrates that office-based plastic surgery procedures in Northern Ontario can be performed safely and effi�ciently. Findings support the growing role of ambulatory surgical care, particularly in regions with limited access to hospital-based services. &#13;
Tailored approaches for older patients, along with procedural refinements, can further enhance safety. Strengthening integration with primary &#13;
care may improve efficiency and outcomes. Further multicenter studies are recommended to validate these findings and inform broader &#13;
adoption.</text>

                <text>Background: The Welsh National Health Service (NHS) dental reform programme seeks to move beyond activity-based performance measures &#13;
towards prevention, needs-led care, and fuller use of the dental team’s skill-mix. Prior research explored patient experiences and barriers to access, &#13;
but less is known about public perspectives on what NHS dental services should look like. This study engaged the public in co-identifying priorities &#13;
and recommending solutions to better align reform with patient needs.&#13;
Methods: A qualitative study was conducted using semi-structured interviews (n=35) and one focus group (n=9). Participants were recruited &#13;
via social media, organizational mailing lists, and research networks, and purposively sampled for diversity. Data collection took place virtually &#13;
between November 2023 and May 2024. Transcripts were thematically analysed using NVivo 12. To enhance rigour, 30% of transcripts were &#13;
double-coded, with themes refined collaboratively.&#13;
Results: Three key themes were generated: the architecture of an ideal service, pathways to access and equity, and enabling the patient–provider &#13;
partnership. Participants prioritized timely, affordable and inclusive care delivered by the right professional, supported by effective communication &#13;
and education for self-management. There was broad support for utilizing team’s skill-mix, risk-based recall and clearer information on urgent and &#13;
emergency dental services. Many participants valued digital tools, such as online booking and teledentistry; however, these findings reflect a &#13;
relatively young, digitally engaged sample. Participants stressed that digital options should remain optional, with robust nondigital alternatives to &#13;
ensure equity.&#13;
Conclusions: Participants emphasized the importance of timely, equitable access, inclusive services, clear communication, and support for &#13;
self-management. They were open to flexible care models, including skill-mix and risk-based recall, provided these were underpinned by patient &#13;
education and trust. Digital tools were positively viewed but must be complemented by nondigital pathways to avoid widening inequalities. &#13;
Policymakers should integrate these perspectives into NHS dental reform to strengthen person-centred, equitable care.</text>

                <text>Background: Measures of patient safety culture among healthcare workers can provide important information about the structure and process &#13;
of healthcare systems as well as helping to identify areas in need of improvement.&#13;
Aim and objectives: The aim of this study was to assess the current state of patient safety culture in Omani healthcare institutions. Specific &#13;
objectives were to provide up-to-date information on patient safety culture across all healthcare institutions under the Ministry of Health and to &#13;
identify priority areas for improvement.&#13;
Methods: A cross-sectional survey was conducted using the Hospital Survey of Patient Safety Culture developed by the Agency for Healthcare &#13;
Research and Quality (AHRQ). The online survey targeted all healthcare workers in different healthcare institutions in Oman. SPSS version 25 was &#13;
used to analyze the results according to the AHRQ’s user guide. Results were compared against similar studies in the country, the region, and &#13;
across the world.&#13;
Results: A total of 1551 employees from 11 governorates completed the survey. The highest dimension of patient safety culture was teamwork, &#13;
and the lowest was related to staffing and work pace.&#13;
Conclusion: Overall, the results were modest and comparable with those from other high-income countries. However, some of the dimensions &#13;
scored less positively in comparison with previous studies in Oman. Despite showing positive attitude, several areas in need of improvement &#13;
were identified. Urgent areas for improvement include staffing and work pace, which are universal concerns, as well as error reporting and response &#13;
to it, both of which significantly impact the safety culture. There should be focus on training in all aspects of safety to improve the culture especially &#13;
with the challenge of high turnover of staff. Specific training is required in communication skills especially when dealing with families and when &#13;
disclosing errors. Similar survey must be conducted regularly to identify areas of improvement and encourage the positive safety attitudes. Despite &#13;
having teamwork as the highest positive response, it is recommended to continuously monitor and enhance it as it remains a critical aspect of &#13;
patient safety culture. This can be done through regular training and fostering of nonpunitive culture and creating a supportive work environment. &#13;
Disparities across organizations highlight the importance of targeted interventions learning from high performers and fostering a culture that &#13;
encourages error reporting and learning from mistakes.</text>

                <text>Background: Healthcare spending in Argentina has risen significantly, yet the system remains inefficient, fragmented, and inequitable, leading &#13;
to poor outcomes and resource waste. Stakeholders often have conflicting priorities, further complicating quality and sustainability. The study &#13;
aimed to identify recommendations to advance the implementation of a value-based healthcare (VBHC) model in the Argentine health system.&#13;
Methods: A modified RAND/UCLA Delphi consensus process was conducted, involving three rounds of expert consultation. The study was &#13;
conducted within the Argentine health system, involving experts from various levels of care (primary, secondary, and tertiary) and sectors (public, &#13;
private, and academic). The geographical focus was Argentina, a middle-income country with a fragmented health system. The Delphi panel &#13;
comprised 21 health sector experts who completed three consultation rounds (two online and one in-person). Evaluation criteria for each recom�mendation included impact, resources, feasibility, acceptability, and measurability. Agreement among experts was also evaluated. The two &#13;
parameters categorized recommendations as appropriate, uncertain, or inappropriate. Exploratory follow-up in-depth interviews were carried out &#13;
to contextualize the findings.&#13;
Results: Twenty-one experts participated. Seven recommendations scored highest in the impact criterion, indicating their potential to advance &#13;
VBHC. These recommendations faced low feasibility and acceptability scores, reflecting systemic unpreparedness. No specific recommendation &#13;
achieved agreement on all criteria with an overall median score above 7, i.e. appropriate. Follow-up interviews highlighted barriers such as policy &#13;
constraints, system fragmentation, political and economic instability, and concerns about sustainability.&#13;
Conclusion: The study underscores the significant interest in transforming the Argentine health system towards VBHC but reveals substantial &#13;
systemic and structural barriers. The findings highlight the need for further research, dialogue, and collaboration to address feasibility and accept�ability challenges. Future studies should explore strategies to overcome policy and structural barriers and the role of political and economic stability &#13;
in enabling VBHC implementation.</text>

ultimately informs decision-making. The Long COVID services within Wales, which form part of the Welsh Government’s ‘Adferiad’ Programme, &#13;
required a rapid evaluation. Patient-reported outcome measures (PROMs) and patient-reported experience measures (PREMs) were used to meet &#13;
the Welsh Government’s prudent and Value-Based healthcare agenda.&#13;
Objectives: To document the benefits and challenges of using PROMs and PREMs as part of a rapid evaluation, in the context of the Long COVID &#13;
service, to provide evidence for decision-makers.&#13;
Methodology: A reflective discussion has been used to explore the benefits and challenges of undertaking a rapid evaluation using patient-reported &#13;
data, which took place between September 2021 and December 2022.&#13;
Results: Facilitators included harnessing cross-organizational working, clear communication, good stakeholder engagement, rapid decision-making, &#13;
and use of validated PROM and PREMs to increase rapidity of roll-out and ability to collect data at a national level. Barriers included the lack of &#13;
information governance permission to collect patient identifiable data, a lack of a comparator group, inability to collect response rate information &#13;
and use of multiple survey platforms.&#13;
Conclusions: Future rapid and standard evaluations, and research in Wales should consider harnessing and developing cross health board working, &#13;
to collect patient identifiable data, response rate information and use a single data collection platform. This would enable the collection of a rich data &#13;
source that could inform national programmes of work. The rapid evaluation of the Long COVID service in Wales provided a useful resource, using &#13;
patient-reported outcomes (PROs) to inform decision-making, and has highlighted valuable opportunities for cross-organizational working in Wales.</text>

                <text>Hospitals worldwide encounter significant challenges in enhancing and maintaining effective incident reporting systems. A fundamental aspect &#13;
of this improvement is fostering the right culture, which facilitates reporting and learning. In this case study, we describe an approach for incident &#13;
reporting improvement at Al Dhafra Hospitals, SEHA, through the conceptualization of a “garden of improvement.” The garden provides a visual�ization of key learning emergent from reported safety incidents to signal to staff the value and importance of their contributions to improving &#13;
patient safety. To address the barriers to effective incident reporting, we employed three key components: a self-reflection tracking sheet, a visual &#13;
depiction of the Garden of Improvement, and a reward system. We observed a 17% increase in incident reporting, including a significant jump in &#13;
near-miss reporting. The proportion of good catch reports also more than doubled, from 3.0% to 6.9%. The implementation of this model has &#13;
supported interdepartmental collaboration, knowledge sharing, and continuous improvement.</text>
