                <text>Background Door to balloon time is a crucial factor of mortality in patients with ST-segment elevation myocardial&#13;

infarction. However, the factors that contribute to failure of achieving door to balloon time≤90 min in an electrocar-&#13;
diogram triage system remain unknown.&#13;
&#13;
Methods This single-center retrospective observational study collected data from consecutive patients with ST-seg-&#13;
ment elevation myocardial infarction from April 2016 to March 2021. The primary outcome was the failure to achieve&#13;
&#13;
door to balloon time≤90 min. A multivariate logistic regression model was performed to predict factors associated&#13;
with failure to achieve door to balloon time≤90 min.&#13;
Results In total, 190 eligible patients were included. Of these, the 139 (73.2%) patients with door to balloon&#13;
&#13;
time≤90 min were significantly younger compared to those with door to balloon time&gt;90 min (p=0.02). How-&#13;
ever, there was no significant difference in sex and timing of hospital arrival between the door to balloon time≤90&#13;
&#13;
and&gt;90 min groups. Presence of chest pain and ambulance usage were significantly more frequent in patients&#13;
with door to balloon time≤90 min (p≤0.01, p=0.02, respectively). Multivariate analysis showed that absence of chest&#13;
pain (adjusted odds ratio 4.76; 95% confidence interval, 2.04–11.1; p&lt;0.01) and non-ambulance usage (adjusted&#13;
odds ratio 3.53; 95% confidence interval, 1.57–7.94; p&lt;0.01) are predictive factors of failure to achieve door to balloon&#13;
time≤90 min.&#13;
&#13;
Conclusion Patients without chest pain as the chief complaint or non-ambulance usage were significantly associ-&#13;
ated with the failure to achieve door to balloon time≤90 min.&#13;

                <text>Carbon monoxide (CO) poisoning is a significant public health issue and a considerable economic burden in devel-&#13;
oped countries. While the majority of non-fire-related CO poisonings are attributed to gas heating, there are several&#13;
&#13;
other less recognized sources that should be considered in the initial differential diagnosis.&#13;
&#13;
The patient in this case was a 21-year-old who experienced a brief episode of loss of consciousness and was subse-&#13;
quently admitted to the Emergency department. Upon evaluation, the patient was diagnosed with CO poisoning,&#13;
&#13;
which necessitated hyperbaric oxygen therapy to mitigate the effects of this toxic exposure.&#13;
&#13;
Despite exhibiting harmful symptoms initially, the patient stated in a phone interview two and a half years post-inci-&#13;
dent that they have not experienced any enduring effects such as cardiac arrhythmia or concentration deficits. While&#13;
&#13;
their understanding of the risks associated with waterpipe smoking has increased, it has not influenced any major&#13;
changes in their waterpipe smoking habits.&#13;

                <text>Objectives ECG interpretation is a life-saving skill in emergency medicine (EM), and a core competency in under‐&#13;
graduate medical curricula; however, confidence for residents/students is low. We developed a novel educational&#13;
intervention—the HEARTS ECG workshop—that provides a systematic approach to ECG interpretation, teaches EM&#13;
residents through the process of teaching medical students and highlights emergency management.&#13;
Methods We used the Kern Approach to Curriculum Development. A review of ECG education literature and a tar‐&#13;
geted needs assessment of local students/residents led to goals and objectives including systematic ECG inter‐&#13;
pretation with clinical relevance. ECGs were selected based on a national consensus of EM program directors&#13;
and categorized into 5 common emergency presentations. The educational strategy included content based&#13;
on HEARTS approach (Heart rate/rhythm, Electrical conduction, Axis, R-wave progression, Tall/small voltages, and ST/T&#13;
changes), and methods including flipped classroom and near-peer teaching. Evaluation and feedback were based&#13;
on the Kirkpatrick program evaluation. The workshop was piloted with 6 junior EM residents and 58 medical students,&#13;
and repeated with nine residents and 68 students from four medical schools.&#13;
Results Residents and students agreed or strongly agreed that the workshop improved their perceived ability (100%&#13;
and 95%, respectively) and confidence (77% and 88%, respectively) in interpreting ECGs. Reports of ECG interpretation&#13;
causing anxiety declined from pre-workshop (61% and 83% respectively) to post-workshop (38% and 37% respec‐&#13;
tively). Residents reported behavior change: 3 months after the workshop, 92.3% reported ongoing use of the HEARTS&#13;
approach clinically and through teaching medical students on shifts. Reported workshop strengths included&#13;
the pre-workshop material, the clinical application, facilitator-to-learner ratio, interactivity, the ease of remembering&#13;
and applying the HEARTS mnemonic, and the iterative application of the approach. Suggested changes included lon‐&#13;
gitudinal sessions with graded difficulty, and allocating more time for introductory material for ease of understanding.&#13;
Conclusion The HEARTS ECG workshop is an innovative pedagogical method that can be adapted for all levels&#13;
of training. Future directions include integration in undergraduate medical and EM residency curricula, and work‐&#13;
shops for physicians to update ECG interpretation skills.&#13;

                <text>Background Ocular complaints, including acute or subacute vision loss, are commonly encountered in emergency&#13;

departments (ED). These potentially time-sensitive complaints are difficult to diagnose and evaluate without ade-&#13;
quate, specialized equipment and expertise. Additionally, a thorough evaluation often requires a more extensive&#13;
&#13;
and specialized physical exam, imaging, and ophthalmologic consultation, all of which may not be readily available&#13;
in the acute setting.&#13;
Case presentation This case report presented a patient in the emergency department with the chief complaint&#13;
&#13;
of vision loss. Point-of-care ultrasound (POCUS) using the 10-MHz-linear-array probe, in the ocular setting, demon-&#13;
strated calcification of the lens, a finding consistent with cataract in the right eye.&#13;
&#13;
Conclusions The use of POCUS can expedite the accurate identification of vision threatening pathology, such&#13;
as cataracts, and streamline ED disposition and plan of care.&#13;

                <text>Background Nowadays, we find ourselves in very unexpected and challenging circumstances facing the COVID-19&#13;
pandemic. The impact of the new coronavirus pandemic probably affected everyone’s mental health, and people&#13;
with pre-existing mental disorders may have an aggravated disease condition, leading to a suicide attempt. Pandemic&#13;
also increased the use of direct-to-consumer telemedicine (TM) exponentially, and consequently, it was expected&#13;
that cases of attempted suicide could be evaluated remotely. Some TM centers have adapted safety protocols&#13;
from psychiatric guidelines for managing these patients. However, there is a lack of evidence of the effectiveness&#13;
&#13;
of follow-up by TM for patients at high risk for suicide, and there is no consensus on what action should be taken vis-&#13;
à-vis the patient who requests immediate help remotely.&#13;
&#13;
Case presentation Here, we reported a case of a TM evaluation of a patient’s suicidal ideation in a direct-to-con-&#13;
sumer telemedicine emergency center, describing the conduct taken in the face of this situation. We also discuss&#13;
&#13;
the importance of planning the emergency telemedicine center for situations of risk of suicide.&#13;
&#13;
Conclusions Telemedicine centers should be prepared for direct consumer assessment of suicidal ideation. Cur-&#13;
rent management suggestions include recognizing the risk profile through institutional training and software&#13;
&#13;
skills and immediate referral for face-to-face assessment, encouraging continuous monitoring until the admission&#13;
and active recruitment of family members or closest friends.</text>

                <text>Background Big cat bites are highly lethal due to the enormous bite force of these animals. This article reviews&#13;
the morphology of these types of injuries and key points of management through a survival case at a Japanese safari&#13;
park.&#13;
Case presentation We report the case of a 26-year-old female keeper who was attacked by a tiger. She was quickly&#13;
transported to our university hospital by ambulance helicopter. The keeper was severely bitten on the head and face&#13;
and had wounds all over her body. Craniofacial repair was performed by emergency surgery. She suffered mild facial&#13;
nerve paralysis and trismus because of being bitten by the tiger and is currently recovering.&#13;
&#13;
Conclusions A multidisciplinary approach of the severe tiger bites successfully treated a young woman cosmeti-&#13;
cally and mentally. Animal farms and zoos that keep tigers should take strict measures to avoid direct confrontation&#13;
&#13;
with tigers.&#13;

Numerous biological systems, such as the cardiovascular, blood-forming organs, liver, and kidneys, can be affected by gas-oline and smoking. Because filling station employees, repair service workers, gasoline truck drivers, and refinery workers are all at a greater risk of being exposed to gasoline fumes. Even though gasoline and smoking have been investigated for so many years, few studies have looked into the effects of gasoline exposure combined with smoking on a variety of physiological mechanisms. As a result, we propose that combining gas-oline exposure with smoking is a risk factor for cardiovascular diseases and impaired hepatic function.</text>

for patients with musculoskeletal injuries, including the Canadian CT Head Rule, Canadian C-Spine Rule, National&#13;
Emergency X-Radiography Utilization Study (NEXUS) guideline, Ottawa Ankle Rules and Ottawa Knee Rules. However,&#13;
it is unclear to what extent clinicians are aware of the rules and are using these five rules in practice.&#13;
Objective To determine the proportion of clinicians that are aware of five imaging decision rules and the proportion&#13;
that use them in practice.&#13;
Design Systematic review.&#13;
Methods This was a systematic review conducted in accordance with the ‘Preferred reporting items for systematic&#13;
reviews and meta-analyses’ (PRISMA) statement. We performed searches in MEDLINE (via Ovid), CINAHL (via EBSCO),&#13;
EMBASE (via Ovid), Cochrane Central Register of Controlled Trials (CENTRAL), Web of Science and Scopus databases&#13;
to identify observational and experimental studies with data on the following outcomes among clinicians related&#13;
&#13;
to five validated imaging decision rules: awareness, use, attitudes, knowledge, and barriers and facilitators to imple-&#13;
mentation. Where possible, we pooled data using medians to summarise these outcomes.&#13;
&#13;
Results We included 39 studies. Studies were conducted in 15 countries (e.g. the USA, Canada, the UK, Australasia,&#13;
&#13;
New Zealand) and included various clinician types (e.g. emergency physicians, emergency nurses and nurse practi-&#13;
tioners). Among the five decision rules, clinicians’ awareness was highest for the Canadian C-Spine Rule (84%, n=3&#13;
&#13;
studies) and lowest for the Ottawa Knee Rules (18%, n=2). Clinicians’ use was highest for NEXUS (median percentage&#13;
ranging from 7 to 77%, n=4) followed by Canadian C-Spine Rule (56–71%, n=7 studies) and lowest for the Ottawa&#13;
Knee Rules which ranged from 18 to 58% (n=4).&#13;
&#13;
Conclusion Our results suggest that awareness of the five imaging decision rules is low. Changing clinicians’ atti-&#13;
tudes and knowledge towards these decision rules and addressing barriers to their implementation could increase&#13;
&#13;
use.&#13;

                <text>Benzene  has  long  been  recognized  as  highly  carcinogenic  and  the most  cytotoxic  of  all  air  pollutants  released  by  gas  stations.  Although  several studies have been conducted on accidents in the process industry, very little work has been directed toward the modeling of risks caused by the leakage and explosion of toxic substances in gas stations. This knowledge could aid in predicting the vapor concentration  inside  gas  station  office  buildings  and  neighboring  infrastructures and in developing corresponding safety measures.                 <text>Nepal is a landlocked country with variable topography including the world’s tallest mountains, tropical forests,&#13;
and Gangetic plains. This topography leads to diverse healthcare needs, from tropical diseases in plains, to road traffic&#13;
&#13;
accidents in hills, and to disaster-related emergencies from floods and landslides during monsoon season and unpre-&#13;
dictable earthquakes. Emergency medicine care is underdeveloped and is currently being provided by general practi-&#13;
tioners. The capital city Kathmandu has a few tertiary healthcare centers with emergency medicine fellowship-trained&#13;
&#13;
general practitioners staffing them, but most of the hospitals outside of Kathmandu are staffed by minimally trained&#13;
or untrained medical officers. Even though emergency medicine is recognized as a specialty, Nepal still does not have&#13;
an emergency medicine residency training program. To improve emergency care in the country, the government&#13;
of Nepal should be focused on facilitating the start of emergency medicine residency in Nepal.&#13;
