The need for certification of safety investigators and learning reviewers in Scotland’s health service (FRONTIERS OF IMPROVEMENT)
Subject
patient safety, investigation, learning reviews, certification
Description
In this short opinion piece, we argue for the ‘professionalization’ of those in the healthcare workforce who lead on safety
investigations and learning reviews. Healthcare is arguably
one of the most complex, safety-critical work systems to have
ever existed [1]. Yet, in considering system safety generally,
and investigation and learning reviews more specifically, it frequently lags behind how other high-risk industries approach, investigate, and learn in their own complex systems [2, 3].
