Patients with congestive heart failure experience symptoms such as shortness of breath, dry cough, and
swollen legs if there is too much fluid in their bodies. A nursing intervention to reduce shortness of
breath in patients with congestive heart failure is sleep positioning at a 45° incline. This case report
aims to determine the effectiveness of 45° sleeping position therapy in patients with congestive heart
failure and shortness of breath. A female patient (38 years old) was treated with complaints of shortness
of breath and complications of obesity. The nurse implemented a 45° semi-Fowler's position with log
roll technique for 3 days. Positioning at a 45° incline can reduce oxygen consumption, increase
maximum lung expansion, and address impaired gas exchange. The log roll technique is very beneficial
for managing and assisting the mobilization of obese patients. The patient's shortness of breath and
oxygen saturation improved after being positioned in the 45° semi-Fowler's position, also aided by
physiotherapy. The log roll technique should be performed with at least four helpers to minimize the
risk of injury to the helpers. Further research is needed to explore the benefits of the 45° sleeping
position intervention combined with the log roll technique in various cases.
