DEVELOPMENT OF MODALITY PULSED ELECTROMAGNETIC STIMULATION (PEMS) TOWARD URIC ACID LEVEL, PAIN AND RANGE OF MOTION JOINT EXSTREMITY FOR KLIEN WITH GOUT ARTHRITIS

Description
Background: Gouty arthritis clients experience barriers to daily activities, walking, bathing, dressing, and limited mobility. The problem of noncompliance with diet, eating habits that are high in purine can increase the build up of uric acid in the joints, then the joints lack of fluids and bone erosion occurs between the joints, causing pain, swelling, joint stiffness, and even bone damage. The purpose of this study was to develop PEMS device and identify its effects on uric acid levels, the degree of pain, and range of motion of the extremity gout client's arthritis.
Method: This research was divided into 2 stages, namely the stage of developing PEMS device and the validation stage of PEMS device applications. Phase one research design used Research and Development (R&D) and phase two used true - experimental pre test - post test design with control group. The study population was all arthritis gout clients in the Kajen Community Health Center, Pekalongan Regency. There were 40 subjects. They were randomly selected and divided into two groups: 20 subjects in the treatment group (PEMS application) and 20 subjects in the control group (given a hot compress). Data were analyzed using paired t-test, Wilcoxon test and Mann Whitney.
Result: the results of the study showed that PEMS modality intervention had an effect on decreasing uric acid levels (p <0.0001), reducing the degree of pain but there was no significant difference between the treatment and control groups (p = 0.25), and effectively increasing the range of motion of the shoulder joint (p < 0.0001) and range of motion of the knee joint (p <0.0001) in gouty arthritis clients.
Conclusion: Modality of PEMS can reduce uric acid levels, reduce the degree of pain, and increase the range of motion of the shoulder joint and knee joint. The PEMS device should be applied as an independent nursing
intervention in stimulating the exuviation of elevated uric acid, reducing pain and preventing joint stiffness of client gout arthritis.
