It is common for people with Low Back Pain (LBP) to seek care in
emergency departments (ED). In Australia, LBP was ranked in the top
five reasons for attending an ED, with over 145,000 presentations
annually [1]. Care for people with LBP in ED is also associated with
considerable cost. In 2019 the calendar year estimated cost to Australian
hospitals for ED care and related hospital admission for non-serious LBP
was around $392.9 million [2].
LBP guidelines are universal in their recommendation that people
presenting with LBP undergo an initial diagnostic triage [3]. Under this
model, people with LBP are assessed and placed into one of three broad
categories. 1. Back pain potentially associated with a serious medical
condition such as fracture, malignancy, or infection. 2. Back pain associated with radicular pain, often coupled with a specific structural pathology such as herniation of the intervertebral disc or spinal stenosis. 3.
Non-specific low back pain (NSLBP), which essentially includes people
that do not fit into the first two categories, indicating there is little
suspicion that there is any specific or serious underlying pathology and
no indication for imaging or further diagnostic work up [4–7]. In
Australia, more than 85% of patients presenting with LBP to the ED were
diagnosed with NSLBP [
